Please note: We are following Sacramento County Guidelines.  

Wait List Request

Tier 1,2, & 3: Are you a healthcare employee or professional (Do you work in one of the following: hospital, long-term care facilities, outpatient clinic, home health care, pharmacy, emergency medical service, or public health)?
Tier 1 Phase 1b: Are you Person aged 65 years or older, Law enforcement and emergency services, Child care, Educator, Food and Agriculture?
Tier 2 Phase 1b: Are you in a congregate setting with outbreak risk, such as those who are incarcerated or experiening homelessness?
Starting March 15th: Persons 16-64 years with high risk medical conditions: (i.e. cancer, chronic kidney disease (stage 4 or greater), COPD, diabetes , heart conditions such as heart failure, coronary artery disease, or cardiomyopathies (but not hypertension), obesity (BMI > 40 kg/m2), pregnancy, sickle cell disease, and/or Solid organ transplant)
